no upload files #541

What steps will reproduce the problem?
1.when I try to upload file does not allow. say "Please complete the following 
information to finish the uploading process. Remember that "Title" is a 
required field." and title is right taken to files?

What is the expected output? What do you see instead?
no upload file.see next image

What version of the product are you using? On what operating system?
r582 any machine on debian 8

Please provide any additional information below.
in version r561 works fine, y need update for security.

problem solved. 
change the permission of folder  /upload/files to 777
