Add support for playback using Yamaha OPL2 #140

Closes #79

detect and initialize OPL2 under DOS
support basic single-voice per channel, 6-channel playback with OPL2 under DOS
map MIDI percussion into OPL2 percussion mode

Other fixes:

fix meta event handling and program initialization for MPU-401
prevent integer overflow in ZIP CRC verification under DOS
convert demo MIDI to Format 0
