Refresh token #252

Motivation

Add the refresh_token flow to the auth0 simulator.

Approach

Return an encoded refresh_token key from oauth/token if the grant_type is refresh_token or the scope includes offline_access.

The encoded string contains the user.id and the nonce key that can be used to look up the user and use the nonce so that new access_token and id_tokens can be created.
