Replace all code & doc references to herokuapp with CNAME #840

A canonical name (CNAME) is a type of Domain Name System (DNS) database record that indicates that a domain name is the nickname or alias for another domain name. The CNAME (https://odop.springdesignsoftware.org) is established as part of the SpringDesignSoftware.org domain configuration at {Hover](https://www.hover.com/). A parallel CNAME is established for https://odop-staging.springdesignsoftware.org

Part of the motivation for the CNAME is support of Google's indexing of SpringDesignSoftware.org.

References in both code and documentation should be made to the CNAME rather than a hard-coded reference to herokuapp. So, resolution of this issue involves a global replacement of: https://odop.herokuapp.com with https://odop.springdesignsoftware.org
