e.g. all ERSPC variables (and answers) as custom concepts.

This makes it more visible which variables are available per source. If we also add Maps to relations, then it also makes the mapping to OMOP more transparent.

Note: might require to map every variable/value pair to either observation (categorical) or measurement (numeric).
