Add better moderator-action logging and usernotes #45

Right now there's no way to tell which moderator banned someone. On Reddit, this is a separate system from usernotes. Here, we should integrate them. My mental image is:

A way to list actions performed on a user, with moderator name and optional explanatory text
A way to list actions performed by a mod (sharing explanatory text)
A way to list all actions recently, ordered chronologically

This is going to be a pretty big project since it involves at least one new database table.

Note: This seems like the kind of thing tildes might have added since we forked. Doublecheck there first to make sure we're not duplicating effort (unless we've already updated.)
