Rework homepage and pattern index layout
Change navigation on homepage and add navigation to pattern index
Change site name and update page titles
Remove standards guidebook reading and references on about page
Fix all of the links on the worked example
Ensure that markdown in problem, context and solution properties are properly processed
Added missing homepage image

Great. Thanks. Resolved a few minor conflicts (eg. had also added latest homepage image this morning)
