When we use the per-chapter headers, it's currently rendering in a confusing way to make it unclear who is responsible for what content.
My current idea is a custom paragraph offset in some way at the top: Author(s) of the "Foo Bar with Baz" chapter:: name , ...
Then it is clear what it is referring to.
It may be possible with the ADOC header format from here:
