The template and VBA commands have different names for several exams. For example, the template now labels that "Upper Extremity Vascular Exam", with the highlighted field of "Exam Cardiovascular Upper Extremity". The second name is what gets reported to SLC when it's completed. However, the Dragon VBA has told SLC to expect "Upper Extremity BP Equality Exam" which is what's displayed in the dashboard. I'm going to branch and make this consistent.
This means the following 2 changes to the VBA for "Chest pain over 40":
"Upper Extremity BP Equality Exam" -> "Cardiovascular Upper Extremity Exam"
"Calf Exam" -> "Musculoskeletal Calf Exam"
This should be the most contained fix. If we decide on different names, we'll need to remember to change them in both places, and possibly update dialogs to match.
