Improvements to Birthday change request form

After discussion with WDPC and previous WRT opinions, there are some changes that would benefit this form.

[ ] Have fill in information for all personal information fields. Full Name, Birthday, Gender, Citizenship, and WCAID (not changeable, but to link the person).
[ ] Ask which fields need correction. So it may be any combination of the first four in the list above.
[ ] For name, ask if the previous name was originally correct and has now changed or if it has always been wrong on WCA.
[ ] For citizenship, ask similar questions to what is in the crash course:

Changes requests in country are particularly delicate as they affects rankings and possibly records. Therefore, such requests must include the following:

Documentation of the old nationality (if available)

Documentation of the new nationality

A thorough description of the change request including answers to the following questions:

Does the competitor still hold the old nationality (as a dual nationality)?

If not: When did the competitor lose the old nationality? Did the competitor ever hold the old nationality?

When did the competitor obtain the new nationality?

Note: residing in a country is not equivalent to having citizenship for that country.`

[ ] add a note that you may officiate any information on the official documentation that is not any of the information we are asking for.
[ ] add a note that if you do not wish to provide documentation, you will need to provide it in person to a Delegate at your next competition and request any changes through the Delegate, but this will take significantly longer.
[ ] Make the form more accessible. Ie: adding a link to it on the edit profile page
[ ] update title, link, and text to not be specific to dob changes.
[ ] Add a note to the email sent that once the issue has been resolved, all parties should delete the document from their mailboxes as soon as possible. - OR - instead have the document be uploaded to WCA, similar to avatars, where only WCA staff are able to view it. Only WRT members are able to delete (this will allow us to be able to send the document to Delegates if we need assistance).
