The README.md is not working, sync server instance section with demo project

This change is not required anymore (and even discouraged). The launcher supports polka & express out of the box: https://github.com/thgh/vercel-sapper/blob/d2a5a50c9e79aa7cdf164a94bbce294a4f674a1b/lib/launcher.js#L26
