What are you running? This file is the one historically for RHEL5, but I see that it gets used for non-RHEL operating systems too, which is probably what needs to be fixed, instead of breaking the module when used on RHEL5.
I'm running RHEL5, and this works fine. I think 5 shipped with postfix 2.3.x?
Specifically, I'm running CentOS 5.8.
Yup, 2.3.3: http://vault.centos.org/5.0/os/x86_64/CentOS/ postfix-2.3.3-2.x86_64.rpm
And, from Postfix: http://www.postfix.org/postconf.5.html#fallback_relay "With Postfix 2.3 this parameter is renamed to smtp_fallback_relay"
OK, interesting since I'm quite sure that I have based that template on the file that RHEL5 installed by default.
Updated in f03e7da97e0ae53b54424979f55741c21a683593 (the file name has changed with my changes from yesterday).
2.2 is really old, y'all :)
