Move notification html files; re-add PWA token location field #616

Proposed changes

Since notification.html was previously a manually edited file, we want to make it clear that changes should go in the mjml file.

A previous addition to the notification template for PWA tokens was wiped when the file was regenerated, so this is re-added here.
