Bug - "Template not found" error is displayed after clicking the Publish button

Steps to Reproduce

1. Loge in as a surveyor
2. Click on the button to create a new survey
3. Complete all required fields
4. Click on the Publish button

Expected Result

• The survey questionnaire is successfully published
• The homepage is displayed showing the newly published survey
• Survey status is set to Active

Actual Result

• "Template not found" error banner is displayed
• Survey is not published
• Publish button is disabled
