Remove link bounding box by using SVG xlink #12

Changed all of the anchors to use xlink-href instead of href, and nested within svg element, rather than wrap them.

Appreciate it (really!), but I'd rather keep the demo code the same as the code people are copy and pasting... the xlinks are great, but I don't think it'll appeal to everyone (from beginners to expert).
