Implement Title/Heading element #54

There is confusion about the meaning of the Head element: transcribers are using it sometimes for titles and headings and sometimes for text at the top of the page.

Titles are part of the main text and can be of different kinds: invocation, chapter title, section title, subtitle, and so on.
