Hi, its nice to find some understanding! I know these problems are not major but they still contribute to the atmosphere. Tynan does not recognise cancer as disease, only as ailment. I also must say that I mostly play with the limited technology reaching 19th century at most. But I'm very far from coding, I don't know how modders implement those limits like a total lethal disease in medieval gets an absolute joke with glitterworld (or how it is called) high-tech medicine. Or they don't...
About carcinoma, it should be a grave disease, limiting a pawns life to one year at best without tech.
Hair loss sounds a bit too much, I suspect it is not easy to code and it is anyways not permanent. Hair grows back anyways if a treatment was successful. This is an issue too minor imho.
Metastases is a very nice idea, could be with 50%50 chance to occur and chance raising higher if a pawn did not get adequate treatment in time (in in-game days, not hours).
Also for you to know cancer needs extra sterile conditions like Tynan did for infections. A pawn could grow "extra low immunity" trait while undergoing treatment. Could be a nice little feature since sometimes an ill impatient pawn leaves hospital for a smoke or lunch.
And you are right, treatment takes lots of time. For example there could be several periods of treatment combined with periods with capability to work (with limitations like excluding some types of work and/or mood debuffs or sick feeling but still capable). Like several days lying in hospital getting chemo and then working and then chemo again, repeat for a year or something.
But actually I'm not sure that people would like this cause its too realistic. So just some ideas for your inspiration.
Cancer is a pretty nasty ordeal. But in rimworld it can be fixed with taping some Herbal medicine over someone's \<insert body part>. Let's change that.
