Updates to Installing section of README.md #173

Trying to find this module in HACS is a little hard as it's under lovelace-card-mod not card-mod as the title suggests. I've updated the install instructions to reflect this.

Also changed the heading to v3 from 3 as it shows that it's a version of the card and again reduces confusion for people trying to search for it in HACS

Also moved all the shields to the top of the page and added a few extra's

As you may have noticed, I'm not very good at keeping this up to date, so I'd like to put as little information as possible which can get outdated in the readme. Better to refer to HACS directly for instructions.
