Please try if the latest beta on the releases page (https://github.com/thorwe/CrossTalk/releases) changes anything.
I deleted the old one and put the new beta on that you wanted me to try, still have sound coming out of both ear pieces and the radio fx is not working still. I have TS run as admin, I have tried it in compatibility mode as well. I also tried older versions of TS before returning to the newest one.
Information that might help, using asus xonar dg for sound card with g4me one headset. Cross talk was working before I upgraded to windows 10 with this software, I also tried to go back to 1.6.1 because other clan mates said they had that working on windows 10 but that did not work for me also.
Also checked to see if channels had surround sound positioning, that does work I can move people around in the channel map and they do move from speaker to speaker.
I want to thank you for looking into this, its a huge pain when its not working correctly.

So after I configured my sound card appropriately everything's working here without any code changes.
The code for anything except PositionalAudio is entirely plattform independent, so stuff stopping to work with an windows update seems quite unlikely - except if TeamSpeak's plugin API gets broken in some way. And as I can confirm it working here on Win10, I'd deem it likely it's in fact your sound card configuration, just like with me.
The Position Spread module has one achilles verse, or well, at least one. If the sound card driver selected as playback device has more than two outputs, it won't work. I'll be looking into supporting higher channel counts.
However, in the mean time, you might try if you can use this tidbit to find a setup problem maybe. E.G. the selected audio out might be in surround mode, despite you wearing headphones. That'd be one thing for drivers / windows playback options.
Why the radio fx aren't working for you, I don't have the slightest idea currently.
Still have the issue, could be that asus does not have drivers for windows 10 for the xonar card. Had to use one of the built ones from other users. I will continue to try more things. Thank you for your help.

For me those issues disappear with TS beta (2) 3.0.18. I'm guessing it's a bug of TS Client 3.0.17 either by itself or in combo with Win10.
For the beta, that's options -> Application -> Update Channel or you wait for the final release. Can you confirm?

Interims heads-up, I think I have stumbled upon an issue with stereo positioning that would cause it not to work on stereo headphones (only). Beta channel update fixing that should be out this weekend at the latest.
https://github.com/thorwe/CrossTalk/releases/tag/v1.6.3.102001-beta contains aforementioned fix. Please let me know if there are remaining issues.
EDIT: Windows 10 just derped in assigning the default program to open TS3 plugins with.
He shall make sure he uses the latest version here from the github releases page: https://github.com/thorwe/CrossTalk/releases The version on the teamspeak addon page is heavily outdated, I could imagine for that to happen with newer TeamSpeak clients. Note that the plugin not showing up otherwise is likely a problem with the addon installer on his computer. There should be two crosstalk*.dlls in his teamspeak_folder/plugins. If they are not there, and he cannot make the plugin installer put them there: .ts3plugin files are simply zip files, he can manually extract it.
Easy-install, Windows 10. Open TS3 folder, and drop the plugin on package_inst.exe. Finish.
I upgraded to windows 10 and now this addon does not work correctly, before I had audio going to 1 ear while having the radio fx. I checked all setting and they are all still what I had set before but now the position and fx are not working.
