Implement complete "Thoth flavour" ONIX 3.0 output #570

Fixes #441.

Also fixes #401.

Notes on Thoth fields not supported by ONIX data model (and therefore not included in this implementation) can be found under the issue and in NextCloud.

I'll also put them in NextCloud if I can find a sensible subfolder for them (suggestions welcome).
