Add react-navigation #3

Sets up the app using a default StackNavigator.
Reorganises around "screens" (a concept from react-navigation).
Applies some sweet Ralph style.

Currently depends on #1, primarily for the starting app structure it provides. Would love feedback on absolutely anything here, perhaps mostly around whether "screens" is the way we want to organise our components.

Perhaps the default title should be {{ cookiecutter.project_name }} rather than the decidedly more boring "Counter".

Update: split the counter and welcome into separate screens in a tab bar controller.
