What is your media type (HDD, USB Flash, SD/SDHC)?

How is your media formatted (wbfs, fat32, ntfs)?

Which cios are you using, which version, which base (222,249,...)?
(Please try both 249 and 222 with cfg-222.dol)

Does it work with other loaders?
don't use others

Does it work with the latest or an older version?
not to my knowledge

Please provide any additional information below.

Can anyone tell about D2x IOS and how to install it (if suggested) to fix games 
like Metroid Prime Trilogy and Sam and Max?

yeah eith eruse modmii to install it or the d2x installer :)

installer is in this thread and it works just like the waninkoko and hermes ios 
installers v6 installer is at version 2.2 now i think if thread does not have 
most uptodate installer

grab it from here isntead

You can install it also with PimpMyWii (but use it carefully 'cause it can 
change also other things and *CAN* brick your wii)

using the version 2.2 with ciosmaps.xml from the downloads page is the latest 
and greatest any other package does not have the latest version with the new 
xml integrated
