[WEB-3023,WEB-3024] Finalize RX permissions #1415

WEB-3023

This basically serves as a starting point for prescriptions work. It:

  1. Fixes a number of css/styling issues that arose with the change over to Theme-UI
  2. Adds the clinic workspace header and makes it generally more consistent in layout with the rest of the clinic workspace UI
  3. Moves from using an ENV var to LaunchDarkly to enable UI
  4. Updates the final prescription state after saving depending on whether or not a clinician has the prescriber role
