missing annotations on some segments of Carelink temp basals

Seems like the first and last segments of a CareLink stepped temp basal are missing the annotation:

Although perhaps we only want the annotation on the first segment in any case? It'd be less cluttered that way...

This is happening because the annotations are created when carelink didn't provide us with the basal rate change, instead we generated it based on our understanding of the schedule. For the first and last segments, carelink does actually provide us with those values, so they can be trusted.
