It should be easier to navigate from one care team to another #80

Today, to navigate from one care team I'm on to another, I have to click the blip logo. This does not feel like an intuitive approach. Took me a couple minutes to figure out.

Indeed, I had in mind that we should add a link in the navbar at some point (top right?).

@skrugman If we go with that, might need an icon for it, or maybe you already have one designed? A "home" icon, or a "people" icon, or something that represents a "list" (although that can be confused with a hamburger nav icon)..?

Is this in Notes app? I need to get a careteam set up so I can see to. Just asked Cheddar to do that for me. We do have a careteam icon - im getting the icon font fixed today and it will be in there @nicolashery
