Example with CVA #35

@timdeschryver got it working now with CVA example, works like a charm.

Only two things missing:

typing of the address field
somehow get the errors through to the parent (singal) form

Thanks @mattlocker ! I'd like to close this one in favor of #36, as that one uses a more signal-driven form as the CVA implementation. It also includes some TODOs that need to be addressed in order to be fulfill all features.
