This PR is bumping the chart version to 12.0.0 as going forward we plan to use versioning closer to one known from kube-prometheus-stack project.
With this PR we are also setting up and automation required to release on every merge
New automation was tested on my fork and resulted in an expected release as seen in https://github.com/paulfantom/tobs/releases/tag/12.0.0
Additionally, I took this as an opportunity to add more metadata to Chart.yaml file and adjust PR template to be aligned with new process.
