Refactor the HDMI interface code with the following aims;

Share more code between input and output parts.
Support multiple different types of "phy" interfaces.
Improve the quality / features supported.
Improve the testing.
Figure out a way to do "emulation".

This issue is more of a high level tracking of the progress.

I'm assuming most of the work will probably end up getting done by @enjoy-digital -- but it would be nice to get other people to understand this code and help improve it.
