This is now working - see https://github.com/mithro/ixo-usb-jtag
Install sdcc, clone the repo and then HARDWARE=hw_opsis make
and load the usbjtag.hex onto the FX2 (using saying fxload) and then set cable type to cable UsbBlaster
in urjtag.
The urjtag STEPPING file for the xc6slx45t also needs to be patched;

Our JTAG interface in on Port E, not on Port D. Sadly Port E isn't bit accessible - so the firmware needs to be modified, not just pins remapped.
