README.md links broken #49

Hey there, looks like some of the readme.md links may be broken. The "How To" link gives a 404, but maybe should point to https://timvink.github.io/mkdocs-table-reader-plugin/howto/customize_tables/.

It's possible there was a single page before, but now it looks like there are three subpages without a common index.
