Initial attempt at cleaning up the spec #29

This is roughly based on #23 with the goal of cleaning up the spec rather than replacing it. I'd like to get this (or a similar PR) in before making any large spec changes so we can easily tell changes apart from cleanup.

There should be no functional changes to the spec with this PR.

I've always assumed base16, but we haven't been very consistent about it lately.
