Add instructions required for base16-shell themes to work in tmux #40

Basically moving the information initially mentioned in this issue into the readme and I'll close the issue afterwards https://github.com/tinted-theming/base16-shell/issues/1

My 2 cents here, this information should be put in the Installation section of readme since its required for the themes to work at all and to be changed through shell variables, similarly how Oh My Zsh requires extra configuration.
