condition type="request-filename" is not documented #171

The only documentation that I can find for this condition type is a changelog 
entry saying that it was added + source code.

While I could just about get along without the documentation, when running into 
problems it would be useful to have the documentation to sanity check (I am 
running into this fatal problem which makes it unusable for me:
