Patients summary page #35

When selecting a patient the summary page is the default. I think that it
could be a great deal more concise as a "summary". 

A table of all the "types" with a last entry date, number of entries and
"list" (all of that type) link might be better. Additionally a separate
show all button to get to a sortable list of everything.

The Summary Page currentlly contains the most recent entry in each of the 
'special' narratives, as defined in the 
program, with a link to see others.
I'd keep it like that till we can get a user-selecteable set of standard 
'views'.
