This PR contains a number of fixes for my T460. This includes fixing HDMI audio, using code I found in the insanelymac thread. This also fixes BT on my DW1560, by using the new kexts found here (https://github.com/acidanthera/AirportBrcmFixup). Finally I've removed the SD kext I PR'ed a few weeks ago: the reader gets recognised but the transfer speeds are terrible, and after sleeping the system doesn't find the reader anymore, so I've removed the kext from my machine.
