Things that need to be improved

[ ] document title should just be read from the title tag
[ ] print layout is all kinds of screwed up
- [x] font too big
- [ ] header doesn't show
- [x] the grid needs to be unset for print so that page breaks work (grid doesn't seem to work together with printing)
- [x] it should be allowed to have a page break within a paragraph
[ ] byline is forcefully added
[ ] layout for more narrow screen should not be asymmetrical
[ ] author positioning needs to be fixed
- [ ] author should be centered if there is only one
- [ ] positioning needs to be done without grid, because the print mode does not like grid
[x] render.js adds back the distill header and footer
[x] font size in general
- [x] the font is the browser is probably too big (it just appears smaller on my screen)
- [x] the font size of article should be 1.0rem because it constitutes the bulk of text
- [x] headings probably should be smaller with respect to the main text (the ICLR template has instructions for this, including instructions on spacing before and after the heading)
- [x] the size of the document title should probably not change with screen size
[ ] instead of <d-cite key="kingma2017"></d-cite>, it should be <cite>kingma2017</cite>
[x] <caption> in <table> should automatically be prefixed with "Table ?:"
[x] use pretty quotation marks: q::before { content: "“"; }
[x] the different grid configurations should be guarded with "screen": @media screen and (min-width: 1000px)
[ ] if there is no appendix, it is generated and produces a horizontal rule even if it's empty

[ ] h3 should probably be included in the ToC
[ ] the amount of space between the number and the heading text should be the same for all heading levels (probably all like it is for h1 right now)
[x] formatting of references

[x] don't use <head> at all and don't close </body> or </html>
- [x] don't close p, td, tr, li, ...

I think I should use <header> instead of <d-title>. and <nav toc> instead of <d-toc>.

Maybe don't use <main> instead of <article>, because <main> should encompass all content-like things which in my case is basically everything. Furthermore, <article> has more semantic meaning in the HTML spec. For example, for the <address> element, it specifically says it applies to the nearest <article> element. This does not work with <main>.
