Remove the setup() method of `PySim` and `Sim`

I believe the setup methods PySim.setup() and Sim::setup() are unnecessary.
The work they perform can either be done in Sim::add_disc() or when Sim::advance() is first called.
Removing the setup method would make things more user friendly as it must be called before running the simulation and is easy to forget

Note this will break all the example code so that will have to be updated.
