update html5boilerplate, bower_components, use_strict, etc.

Include HTML5 Boilerplate and update index.html to be consistent with the original angular-seed app
Move bower_components into the /app folder
Move use strict; statements to top of JS files so they apply to the entire file, not just their functions
Bump versions on karma-requirejs and karma-coffee-preprocessor
Cleaned up .gitignore files
All unit tests and end-to-end tests execute successfully
