Reverts toladata/TolaActivity#1139

We need to bring address back. We broke Kupfer frontend because they are using it.

Next time, before deleting a field, please use git blame and see who added the fields and don't merge until the author approves. Also it's my fault for not revising the PR after merge, but you know how busy we normally are ☹️
