Headers for backmatter chapters #22

For the glossary, bibliography, and index the left header is printed in small caps while the right is printed normally. This is fine for the normal chapters where the chapter title is printed on the left and the section tittle is printed on the right, but it looks weird for these backmatter chapters.

Worse, by default \printindex does not update the marks so I had to set them manually with

\clearforchapter
\markboth{\indexname}{\textsc{\indexname}}
\printindex

This might not be an issue and thus not require fixing. Check how other books do this.
