Remove any HTML when generating the table of contents #63

When a h1-h6 header contains a link, the table of contents plugin does not strip it out, and thus generates links that are invalid, such as Current version - v0.2.0">Current version - v0.2.0 which should simply read Current version - v0.2.0.

This was caused by the fact that the TOC text property was in fact the full anchor and not simply a text.
