Player death and respawning

Implements a few behaviours around player death and respawning, as well as ways to keep players out of the game when necessary:

• Players hit by bomb explosions will die. Dead players are fully despawned so they can start again with a fresh wasm state.
• The associated player handle gets put in a special "respawning" state where it will count down until ready to go.
• Misbehaving player handles may be put on a special Panicked state (later on we might want to add a TimedOut one) that keeps them out of the game until the file is removed or replaced.
• I wanted to mess with visibility and timers so I added a fading skeleton on player death :)

Yep, we probably will need to give a bit of thought to sight ranges, explosion timings and other things :smile:
