refreshFilterToolbar doesn't take into account index, only uses name #999

refreshFilterToolbar doesn't look at index in colModel vs filters and thus only looks at fields where name is the same as index, so it doesn't work if you have colModel fields that have index set to a value different than name. Can this be fixed?
