Steps to Reproduce

1. Open the URL http://127.0.0.1:5000/
2. Click 'Create configuration' Tab
3. In ' POPULATION AND DATA SCOPE' section; Select 'War Era' as World War II
4. Click 'NEXT' button
5. From the 'Add Diagnosis' drop down seelct first two items [R41-Other symptoms and signs involving cognitive functions and awareness / C80-Malignant neoplasm without specification of site]
6. Click ADD
7. Click on 'POPULATION AND DATA SCOPE'
8. Change the 'War Era' as Vietnam
9. Click NEXT
10. Click on 'Add Diagnosis' and select first two items [R41-Other symptoms and signs involving cognitive functions and awareness / E11-Type 2 diabetes mellitus]
11. Click ADD
12. Check the list

Actual Result(s)

• Duplicate 'Add Diagnosis' displayed when user add Diagnosis from different War Era
