Support customizing alert levels #10

Hi,

I've modified quite a bit with this one. Including support for modifying the alert levels in a similar way to the Zabbix Pushover integration. Not sure if this is how you want your version to behave, so definitely hard feelings if you don't approve the PR. It was just what I wanted it to do.

Sorry for the late answer. Im going to have to test this alittle when i have time. Need to upgrade my zabbix clusters first and do alittle maintainance ;) It looks good, and its probably a feature more people wants. (Including me)
