Update changelog for 4.0.0 #242

There's a bunch of intervening releases that also aren't listed in CHANGES.rst.

Since you guys apparently aren't in the habit of listing each change in both the GitHub releases and CHANGES.rst, I'd recommend writing them in only one place, so they don't get desynchronized.
