[contact] edit medical context #30

I have just made a stub form that doesn't do anything. The important part is that the clicking on the gears pops up the modal.

We will need to add ajax submissions for most of these forms. We need to rewrite the whole javascript layer and should probably have a base.js for helping with ajax submissions.

Moved this to PreDeploy training so we know to add the backend.
