In the event that an episode is wrong (IS THIS WRONG? in the thumbnail) on the trakt side, it would be nice to have a link to the episode in the streaming site's history that the extension is trying to match.

Usually a wrong match and a best guess effort at resolving.

Nothing concrete. Depends highly on one's watch history and streaming service.
