Addresses point 1 in #69:

Well-formedness error: at line 55272 there is an unescaped ampersand ("&" should be "&").

I see from #69 that this may cause merge issues. Feel free to close without merge.

@lucafavatella: I am going to go ahead and close this without merging. After Monday, 9 January, we should be able to take care of XML and other formatting issues.
