Create an issue template #601

Good/bad idea? I would also welcome suggestions on the proposed template below.

Motivations:

Make the life of contributors easier
Encourage (not enforce!) a standardised issue format

@TheoPannetier I suggest to treat this Issue like all others: just go! If the team complains, we'll undo it.

If you still want a vote, I can arrange that as well :-)
